Thank you for scheduling an appointment with our office. You will find the forms that we will need when you come to your appointment by clicking the link above titled 'Medical Forms To Fill Out'. Just print them, and fill them out and bring them with you when you come to our office. Please make sure you complete these before you come, so that we can begin your evaluation in a timely manner. If you are taking medications, please be prepared with a list of what you take. Your pharmacist will be able to help you with this if your prescriptions are extensive, or you may just bring your prescriptions to your appointment. If you would like to ask your pharmacist to fax a list of your current medications to us, our fax number is (256) 383-9135. Also, if you are coming for an evaluation regarding teeth extractions or a TMJ related problem, a panoramic x-ray is helpful. If your general dentist has taken this type of x-ray within the past year, we can use it here. Please contact your dentist regarding this, and he/she will allow you to pick it up to bring to your appointment.

When you arrive, you will need to go to the second floor to check in. This means that when you go in the front door, you will need to go DOWN one floor either by stairs or elevator. If you forget, follow the signs on the wall for instructions. Your first visit with us is for evaluation only. We do not perform same day evaluations and surgeries unless the doctor, after seeing you, determines that your situation is an emergency. We look forward to seeing your soon.

* Payment is due as services are rendered unless other arrangements are made in advance. If you have insurance, we will gladly process your claim, but we request that you pay your estimated portion when services are rendered. Thank you!
